iCare-PD Annual Meeting 2021

The iCARE-PD consortium is a multinational multidisciplinary research project aiming to develop innovative sustainable care models shifting from an (in)outpatient care model to home-based community centred integrated health care that focus in Integrated Care, Self-management support and Technology-enabled Care using a patient-centred approach.
This meeting is open to anyone interested in contributing to the conversation and will be hosted on May 10th, 7 – 11 a.m. (EDT).
